Ozone vs PRP Injections

Both are regenerative. They work differently, cost very differently, and shine in different situations. Here's the honest breakdown from Dr. Dustin Drussel.

The short version

PRP (platelet-rich plasma) uses your own concentrated blood platelets to deliver a high-dose growth-factor cocktail directly to a damaged tissue. It is the more potent option for advanced or stubborn cases, but it is more expensive ($500 to $1,500 per injection) and rarely covered by insurance. Ozone is the more accessible regenerative option ($125 per injection at our clinic in Orem), works well for soft-tissue and early to mid-stage joint conditions, and is usually a reasonable first regenerative trial before stepping up to PRP.

How each one works

Ozone

Medical ozone is a gas, generated fresh from oxygen and injected into the affected area. It works through three pathways: increased local oxygen utilization, modulation of chronic inflammation, and activation of resident repair cells (fibroblasts, chondrocytes, stem cells). It does not contain any cells or growth factors itself; it tells the existing tissue to wake up and start healing.

PRP

PRP uses your own blood. A small amount is drawn, then spun in a centrifuge to concentrate the platelets and growth factors. The resulting plasma is injected back into the damaged tissue at a much higher concentration of healing molecules than the body would normally deliver on its own. It directly delivers what the body uses to rebuild tissue, in a concentrated dose.

Both stimulate repair, but through different mechanisms. PRP brings in concentrated raw materials. Ozone activates the existing machinery.

How to choose

Start with ozone if

  • You have soft-tissue pain (tendinitis, plantar fasciitis, rotator cuff issues)
  • You have early to mid-stage joint pain that has not yet been imaged as advanced arthritis
  • You want to try a regenerative option without committing $1,000+ up front
  • You want to combine the injection with chiropractic and soft-tissue work in one visit
  • You are exploring options before deciding on something more aggressive

Step up to PRP if

  • You have advanced joint degeneration where ozone may not be enough
  • You have a partial ligament or tendon tear that needs concentrated growth factors to heal
  • You have tried ozone and want a more potent regenerative option
  • You are post-surgical and want to accelerate the healing of repaired tissue
  • You are an athlete with a high-stakes timeline (return-to-play deadlines)

Some patients use both

A common protocol: a series of ozone injections to address overall tissue health, followed by one or two PRP injections targeted at the most stubborn area. The compounding effect can be meaningful, and the lower-cost ozone work usually reduces the total amount of expensive PRP needed.

PRP is available at specialty regenerative medicine clinics and orthopedic offices throughout the Salt Lake and Utah Valley areas. Costs vary widely. We refer patients to PRP providers when their case warrants stepping up.
